Booking Your Appointment

The booking widget is located at the bottom of this page. Please allow a few seconds for it to load—especially if you are using a mobile device. For the best experience, I recommend booking from a desktop.

When scheduling, please select the correct type of visit and make sure to arrive on time, whether your appointment is in-person or via telehealth.

Once you book your appointment, you’ll receive intake forms via email.

Your appointment is not confirmed until these forms are completed and submitted.

 

Telehealth Appointments

You will receive a video appointment link before your scheduled time.
Please note: You must be physically located in the state of Minnesota and not operating a vehicle during your telehealth session.

 

In-Person Appointments

You will also receive a video appointment link before your scheduled time, just in case.

Patient Portal Access

Once you're established as a patient, you'll have access to a secure patient portal, where you can:

• Book and manage future appointments

• Send secure messages

• View and download invoices and superbills

• Complete and sign documents

• Access prescriptions and questionnaires
