-Via telehealth: 

You will get a video appointment invite prior to the appointment. For telehealth visits, you need to be in the state of Minnesota at the time of the appointment and not driving a car.

Once you book your appointment, you will be asked to complete and review important intake forms via email. Your appointment is not confirmed until these forms are submitted.
